First Office Visit

We look forward to your upcoming appointment, in order to make your appointment as productive and as informative as possible, please be sure to bring along the following information:

• Patient registration forms
• Practice consent form
• Policy for prescriptions and forms
• Insurance cards and photo ID
• Previous gynecological records if available (i.e. abnormal pap reports, surgical reports) if available (i.e. abnormal pap reports, surgical reports)
• Radiology reports (i.e. sonogram, MRI, CT-scan) if available
• Summaries or medical records from the referring physician

Please complete the patient registration and history information prior to your arrive for your appointment. Completed paperwork can be faxed to us at 703.717.4009 at any time. If you do not have access to the internet and/or your paperwork has not been completed in advance, please arrive 30 minutes before your appointment time so that the necessary paperwork can be completed in a timely fashion.
